With Alex Delarge it’s a matter of emphasizing the characters youth corrupted by the world around him. Milk in all of these cases has a different symbolic purpose. Alex's milk is drugged, a normalised in-universe beverage as you'd find whiskey at a bar in ours. Clearly stated in the book iirc.
